Consultation and Treatment

Before initiating BioTE® Pellet therapy, patients must complete an initial consultation, including a detailed medical history, physical examination, and necessary lab work. Treatment plans are based on the clinical judgment of our healthcare providers.

Patient Responsibilities

Patients must provide accurate and complete information during consultations and adhere to prescribed follow-up appointments and recommended lifestyle changes. Any changes in health status or adverse reactions should be reported immediately.

Payment and Refunds

Payment for services is due at the time of treatment. We do not offer refunds for HRT services once the treatment has been administered. Patients are responsible for understanding the cost of treatment and any associated follow-up visits.

We are committed to protecting your privacy. All patient information is kept confidential in accordance with HIPAA regulations. Your information will only be shared with authorized personnel directly involved in your care unless otherwise required by law.

Limitation of Liability

Rejuvenate U Wellness Clinic is not liable for any adverse effects or complications arising from the treatment if the patient fails to follow the prescribed regimen or fails to disclose important health information.

We reserve the right to modify these terms and conditions at any time. Any changes will be communicated to patients and will apply to all future treatments.
